How to retrieve lost passport information from 2003

Hello! My wife and I wanted to file a I-485 to change her status. When she was little, her guardians lost her original passport that was used to enter the United States in 2003. We are using Boundless to fill out our form and they are requesting her original entry passport number and expiration date. Since she doesn't have the original passport, is there a way for us to obtain this information through USCIS or other agencies?

We would really appreciate it if someone with a similar experience or knowledge on the situation could help answer our question. Thank you!

DS-260 Have you ever attended any educational institutions at a secondary level or above?

Ok so I’m not sure what to put down here, I have a high school diploma and currently attending college! So for the occupation part for a previous question I put down the school I go to and address, however I am uncertain what to put down here, I put my high school start and end date but do I also include the college? Because it says highest level of education COMPLETED but also in the drop down menu there is an option that says UNIVERSITY,NO DEGREE which if I do choose then I will have to fill out the school information and then a start date and an end date which I still do not know this is the only question I’m struggling will and have looked into it but no straightforward answer!
